

More about the Process
It is time to provide a biography about who you are.
We will compare your biographical information with the biographical information our mentors have provided. We are not looking for perfect matches because we want you to learn new perspectives and have the social ability to connect with people who may not have your same personality type. What we do want to do is connect you with a person who has had and reached some similar goals as those you aspire to, who can introduce you to people you can relate to, or understands those areas of interest that inspire you.
Although you will be providing a lot of information about yourself to a person you have not met, your potential mentoring coach will only know your first name. The information you provided on your HOPE365plus application will not be shared during this process.
You will get a chance to meet your HOPE365plus mentoring coach prior to the final assignment. Remember, we are not looking for a YOU-CLONE, so do not expect the person you meet to be perfect. But do know that the person we introduce to you has been vetted, screened (when necessary), has provided references, and was specifically recommended by a pastor to be a mentoring coach.
Now it is time to complete your biographical survey form. At the end of the survey, you will be asked to write your introduction and request for a mentor. Directly below that section you will find 6 examples of what such an introduction and request might look like. The process of completing the survey will remind you of many things about you that you might use on that last step and we will help edit or condense your submission as needed. Of course we will let you approve any edits before we submit your request.

Examples of Introduction and Request

Don Francis
Am a 46 year-old man from Lake City FL. I will graduate from the Teen Challenge Jacksonville Men's Center in December and will enter the TC Reentry program in January 2026.
I have a BS in marketing from the University of North Carolina and have experience in sales and offset printing. I expect to be employed by Maybery Restaurant Supply as a warehouse worker in Jacksonville FL. My goals are to earn enough money to satisfy the requirements to get a driver's license and then purchase a car. I enjoy cooking, biking, bird watching, and hiking.

Tess Brown
I am Tess and I am a mother of 3 teenagers who are currently living with their dad. When I leave Teen challenge, I will live with my parents while saving money to get my own place. As a homemaker for 17 of my 32 years I have no skill that will help me get a good job, so I plan to go to school to learn how to cut hair.
I was saved 9 months ago while attending a church in Jacksonville, FL. I have given my life to the Lord Jesus Christ and would like to have a mentor who can help me become the best Christian woman and mom I can be. I have a driver's license so I can work, but I will need help finding a job. I am willing to do just about anything to meet my goals.

Kevin Nye
My name is Kevin Nye and I am a 23 year old from Orlando, Florida. I have been in Teen Challenge 2 times over the last 3 years. I would like to change the way I live and learn new ways of dealing with life. I will be staying in Jacksonville and entering the Reentry Program in 3 months. I am currently working on the landscaping crew and I like this kind of work. I will be looking for a company who does this kind of work when I start reentry.
I enjoy cooking, reading, learning new things, like the Bible, and appreciate every opportunity I have to talk with people about the Lord. I plan to be active in serving at church but I do not have a church home yet.

Ashley Jones
I will age out of foster care in July 2025 and will take advantage of tuition assistance to attend college at University of North Florida. I do not know what i want to major in at this time but I know I have time to choose. I would like a mentor who can help me stay focused on the Lord and keep me motivated to remain in school. I have no family that I am aware of, so having a more mature person to keep me aligned with the Bible should be very helpful in keeping me hopeful. I am a diehard Gator and enjoy everything about the beach. My favorite place to meet is in any Coffee shop as I love trying new and unique coffees.

Lisa Rose
I am already in reentry with teen challenge in Columbus Georgia and would like to have a mentor who can help me move to the next place in my life. I am employed with the Teen Challenge Thrift Store, but as a 46 year old woman, it is time for me to look beyond Teen Challenge and become more independent. I have previously worked in real estate and hope to renew my license. My experience is in MDU leasing but I am willing to work in other areas. More than anything, I hope to reconnect with my 19 year old son who is currently engage to be married. I would like to have a Christian mentor who is mature enough to help me be the best mom I can be to my son and his new family.

Alex Young
I am about to age out of foster care and have already been accepted into the music program at Jacksonville University. I will live on campus as a freshman which will be a benefit since I do not have a drivers license, car or a place to live. My future plans are to get my BA in music then go to seminary for worship arts. I plan to serve in ministry as a worship leader.
I am seeking a mentor who can help keep me grounded in the Lord and teach me how to prepare for ordination in the church. Other than music, I enjoy hanging out with friends and spending time with my foster family.
